This is a incoming 6th & 7th grade Life School Elementary (OC, Cedar Hill, Mountain Creek, Lancaster and Carrollton) Summer 7 on 7 FB team. Tryouts will be Saturday, May 6th from 10-12pm. As the parent/guardian of a minor, I hereby grant permission for my student to participate in the camp(s) and acknowledge the fact that he/she is physically able to participate in all camp activities. If in the judgment of any representatives of the school, my student needs immediate care and treatment as a result of any injury or sickness, I do hereby request, authorize, and consent to such care and treatment as may be given to said student by any physician, athletic trainer, nurse, hospital, EMT, or school representative; and I do hereby agree to indemnify and save harmless the school and any school representative from any claim by any person whomsoever on account of such care and treatment of said student. I will not hold the school district financially responsible for the emergency care and/or transportation for said child. I request that the physician, dentists and staff of the medical facility perform any diagnostic procedures, treatment procedures, operative procedures, and x-ray treatments and anesthetics as may be necessary in the diagnosis and treatment of my child.
